ELCHK, Grace Rehabilitation Service
Introduction
Grace Day Activity Centre and Hostel starts providing services from July 1992. It consists of a day activity centre and a hostel serving 52 severely mentally handicapped persons on residential service, training and medical care. We also have “Residential Respite Service (Casual Vacancy)” and “Extended Care Programme”.

Service Objectives
- “Grace Hostel” It provides home living for persons with severe mental handicap who lack basic self-care skills and require assistance in personal and nursing care.
- “Grace Day Activity Centre ” It provides day care and training in daily living skills and simple work skills to mentally handicapped persons. It trains them to become more independent in their daily lives, and prepares them for better integration into the community or for transition to other forms of service or care.
- “Extended Care Programme” For those trainees who could no longer benefit from prolonged or intensive training due to ageing or deterioration of health condition, and to provide simple rehabilitation training as well as social, leisure and developmental activities.
- “Residential Respite Service (Casual Vacancy)” It provides family members/carers of people with disabilities a planned short break so that they may attend to their personal business such as undergoing a medical operation or taking a vacation tour. Its purpose is to make life for those involved in routine care for people with disabilities less stressful and more stimulating.

- Severely mentally handicapped persons, aged 15 or above;
- Actively occupied in or being arranged for admission to a day activity centre;
- Physically and mentally suitable for group living; and
- Without severe aggressive behavior endangering self and others, and without infectious disease;
- Not bed-ridden or requiring infirmary care.
- 50 years old or above and receiving DAC & Hostel service (frail cases aged under 50 may also be considered);
- Assessed by the Standardized Assessment for Residential Services for People with Disabilities to be eligible for HSMH or C&A/SD service.
“Residential Respite Service (Casual Vacancy)”
- People with intellectual or physical disability;
- Aged 15 or above;
- Physically and mentally suitable for communal living and not suffering from behavioural problem or contagious disease; and
- In need of a certain level of personal / nursing care not beyond the level provided by the particular type of residential service.

服務簡介
天恩展能服務於2017-2019年增設嶄新項目「耆智護骨@社區」計劃，為荃葵青區的智障人士及其照顧者提供12個月的跨專業團隊服務。此計劃結合中、西醫學診治、跟進及預防骨質疏鬆症，減低病症帶來的影響；同時建立社區支援網絡；並推廣公眾教育及社區共融活動，喚起大眾認識及預防骨質疏鬆症，為弱勢社群建立健康、充滿愛與關懷的社區。

現時香港沒有醫療政策或社會服務支援智障人士家庭，面對預防及跟進骨質疏鬆症治療的項目。嶄新項目「耆智護骨@社區」計劃，專為智障人士和其照顧者在預防及診治骨質疏鬆症，提供跟進服務及支援。本計劃概念以中醫緩老養生為主線，並加入中、西醫合作診治骨質疏鬆症，給予處方介入性的治療及跟進，並於需要時作出適切的轉介。
除此之外，本計劃成立一支由註冊社工帶領的義工團隊「朋儕支援小組」。透過小組定期舉辦小組聚會/活動，聯絡、關心及支援智障人士家庭，協助這些家庭建立互助小組，共同面對骨質疏鬆症帶來的影響，加強自助互助的效益，讓智障人士家庭認識及善用社區資源。
透過公眾教育宣傳、社區活動及教育單張，推廣公眾人士認識、正視及預防骨質疏鬆症，本計劃不但會建立社區支援弱勢社群的服務網絡；更希望喚起公眾人士對「雙老」及「三老」的智障人士家庭的關注。
