Service Objectives
Providing a package of tailor-made, home-based continuum care services cater for the individual needs of frail elders to meet their physical, psychological and socio-interpersonal needs, allowing them to stay home to spend their old age as they wish. With our diversified services and ancillary facilities, the service also aims at supporting and relieving the stress of their carers/families, enhancing the knowledge and skills and thus the overall nursing quality of the carers.

Target Groups
Residents aged 65 or above living in Sha Tin (aged 60-64 when necessary) who are assessed as frail elders under the Standardised Care Need Assessment Mechanism with moderate to severe levels of impairment; who have to attend follow-up medical consultations regularly; or those who have to receive minimum level of home-based residential and community support services (including nursing, rehabilitation and personal care service).
