Integrated Discharged Support Program for Elderly Patients was established in July 2009. It provides support services for the elderly dischargees of Tuen Mun hospital, and continuity of community health services. The service has been extended to Pok Oi Hospital in 2012/2013.

Service Objectives
-Coordinating with ‘Tuen Mun and Pok Oi Hospital integrated discharge support programme’ to provide community support services for elderly dischargees
-Arrangements for the discharged elderly in need of home-based support and rehabilitation after discharge, as well as transitional support services in order to improve the quality of life of the discharged elderly, and reduce the risk of re-hospitalization
-Training and support to carers to improve the quality of care

Target Groups
-Residents aged 60 or above in Tuen Mun and Yuen Long District
-Referrals from Tuen Mun Hospital and Pok Oi Hospital
- Discharged elders who require rehabilitation, personal care and home-based support service.

服務內容

1.      與屯門醫院及博愛醫院「離院長者綜合支援計劃」通力合作，提供社區支援服務予長者病人。

2.      為有需要的出院長者安排出院後的過渡性家居支援及復服務，以改善出院長者生活質素、減低再次入院風險。

3.      培訓及支援護老者，提升照顧質素及減輕照顧壓力。
